Using Your Health Insurance at Boulder Medical Center
Boulder Medical Center accepts most health insurance plans, and if we don’t, we can submit a one-time claim to your out-of-network insurance provider as a courtesy.
Why We Need Your Insurance Card to Every Visit
When you visit your provider at any Boulder Medical Center office, we will ask for your health insurance card every time. Even if your insurance has not changed since we last saw you, checking your card at each visit helps us ensure your plan will cover your visit as you anticipated (deductibles, co-pays, self-pay, etc.). A printed/card or digital version on your phone works great.
Your Responsibilities
Contracted Insurance Plans (In-Network)
As a patient, you are responsible for knowing if Boulder Medical Center providers are contracted (in-network) with your insurance plan. The best way to confirm this is to contact your insurance company and provide them with Boulder Medical Center’s:
- Tax ID #: 84-0834835 and
- NPI (National Provider Identifier) #: 1306820923
Non-Contracted Insurance Plans (Out-of-Network)
If Boulder Medical Center is not contracted (out-of-network) with your insurance provider, you will receive a service estimate at least five days before your appointment.
- The estimated amount must be paid on the day of your visit.
- The estimate is the minimum amount. There may be additional charges depending on the treatments or procedures received during your appointment.
- You are responsible for the full cost of services and will be billed for any remaining balance.
- If the amount you paid exceeds the final billed charges, we will issue a refund within two weeks of your appointment.
- As a courtesy, we will copy/scan your insurance card and submit a one-time claim to your insurance provider.
