Patient Liaison Services at Boulder Medical Center

Our Patient Liaison Department is available to identify care problems, direct patient inquiries or concerns, and facilitate resolution. We can help patients by:

  • Receiving feedback on your experience at Boulder Medical Center
  • Collecting information regarding concerns about the care that you received or your interactions with providers or staff
  • Act as a liaison for you to providers and/or management

  • Complete our HIPAA-protected Concern & Feedback form so we have a complete record of your perspective on the concern.
  • Call (303) 440-3125 and leave a message describing your concern.
  • Email. Include a brief summary of your feedback. Be sure not to include personal health information.
