Schedule a Colonoscopy or Endoscopy

  • Please call (303) 440-3216. A referral is not required to schedule an appointment, but our scheduling team will assist patients in obtaining the needed referral paperwork.
  • Patients not seeing a Boulder Medical Center Internal Medicine or Family Medicine provider can complete this Registration Form and have your provider fax a referral with recent visit notes to (303) 440-3209. Cardiology patients will be asked to provide cardiology records and/or clearance.
