Requesting Medical Records

Medical Records can only be obtained by Boulder Medical Center patients or their legal representatives. There may be a fee associated with requesting medical records.

Option 1: Request Online

Medical Records from BEFORE September 9, 2024

To request medical records from on or before September 9, 2024, or if you do not have a MyChart account, complete this online form. You will need to have a photo ID and cell phone ready to complete the request. We recommend using Chrome, Firefox, or Safari browsers for the best experience.

Medical Records from AFTER September 9, 2024

To request copies of your medical records from AFTER September 9, 2024, log in to your MyChart account and select “Sharing Hub” from the top left drop-down menu.

Option 2: Complete and Return Form

1: Complete the Medical Record Release Form

Complete and sign a Medical Record Release Form. This will allow Boulder Medical Center to release or obtain your medical records.

2: Return the Form – Choose One Method

After you complete the form, please submit it via:

  • Fax to our document processing company at 303-449-9380. This is the quickest method.
  • Email to releaseofrecords@bouldermedicalcenter.com (Note that emails do not go to a protected site – the Record Release Form contains private/personal information.)

Immunization Records

Use the CIIS Public Portal to request an official copy of your immunization record from the Colorado Department of Public Health.
