MyChart Proxy Access for Family Members
MyChart lets you view and manage a family member’s health records by setting up proxy access. Proxy access is typically granted to parents or guardians of minors, legal guardians for adults, or individuals with healthcare power of attorney. Court-appointed guardians or other legally recognized representatives may also qualify.
Getting Proxy Access for Your Child/Family
Proxy access is required for us to give parents/family access to their child’s/family member’s online account to make appointments, send messages to providers, or review medical records. A completed proxy form MUST be submitted to receive access. Forms are available at our reception desks and are linked below. Please note that granting the proxy access may take a few days after we receive the completed form.
Patients Aged 0-11
- Complete and submit a proxy form for your child(ren).
- If you don’t have your own MyChart account, sign up for one. Access to your child/family member’s records is via your MyChart login.
Patients Aged 12-17
- Existing proxy access is deactivated at midnight on the day of the patient’s 12th birthday, and a new proxy form must be completed for the new age range.
- The CHILD must sign the new proxy form.* This is a State of Colorado mandate, not a BMC requirement. If the parent signs the form, it will be denied.
Patients Aged 18+
- Previous proxy access is deactivated at midnight on the day of the patient’s 18th birthday, and a new proxy form must be completed for the new age range.
- The ADULT CHILD/ADULT must sign the new proxy form.*
*Exceptions can be made for patients with certain physical or mental limitations.

What to Expect After Requesting Proxy Access
- If you don’t have a BMC or BCH MyChart account:
- You’ll receive an activation code via email to set up your MyChart.
- Once your account is set up and proxy access is approved, the patient’s name will appear in the “account switcher” dropdown in the upper right corner of your MyChart account.
- If you already have a Boulder Medical Center MyChart account:
- Once proxy access is approved, the patient’s name appears in the “account switcher” dropdown in the upper right corner of your MyChart account.
