As a Medicare beneficiary, it’s important to understand what’s included in your Medicare Annual Wellness Visit and how it differs from a traditional physical exam. At Boulder Medical Center, we offer these visits to help you focus on preventive care, healthy aging, and long-term wellness planning.

What Is a Medicare Annual Wellness Visit?

A Medicare Annual Wellness Visit is a once-per-year appointment covered by Medicare Part B. Unlike a routine physical or sick visit, the visit focuses on health risk assessment, screening, and prevention—not diagnosing or treating new or existing conditions.

At Boulder Medical Center, your wellness visit is designed to provide a comprehensive overview of your health which helps us identify early warning signs and create a personalized care plan for healthy aging.

What is Covered In the Visit?

During your Medicare Annual Wellness Visit at Boulder Medical Center, your provider may include:

  • Completing a Health Risk Assessment (HRA)
  • Reviewing family and personal medical history
  • Measuring height, weight, and blood pressure
  • Evaluating cognitive function and memory
  • Identifying risk factors for chronic disease
  • Assessing daily living activities and mobility
  • Mental and behavioral health screening
  • Discussing Advanced Care Planning (optional)
  • Developing a personalized preventive care plan

These services are designed to keep you well—not to treat illness or perform diagnostic testing.

What’s Not Covered In the Visit?

It’s important to understand what Medicare does not include as part of your Medicare Annual Wellness Visit. If any of the services listed below are performed during the same appointment, they will be billed separately and may require copayments or meet deductible requirements:

  • Physical exam or hands-on body check
  • Vaccinations or immunizations
  • New prescriptions or medication adjustments
  • Evaluation or treatment of chronic conditions
  • Acute illness or sick visit care
  • Diagnostic services (bloodwork, X-rays, lab tests, etc.)

We recommend scheduling a separate appointment if you need these services to ensure Medicare coverage is applied correctly.
